1. Those services in the BREMSS and Southeast Regions must submit the following plans for review this renewal cycle: Fluid/Drug, CSP (for those who carry controlled substances), and Infectious Disease Prevention. The Biohazard plan is no longer required.
3. If a plan requires signatures (e.g. Controlled Substance Plan), they must get new signatures with dates before submitting the plan.
4. Services with bases in more than one county may submit one set of plans for all locations. Please add a cover page listing the counties in which they apply along with the respective service ID numbers. In order to do this, all locations must follow the submitted plan. This may require adjustment to some of the verbiage to reflect more than one location or perhaps how the medications are handled, stocked, or accounted for to accomplish this, but I hope in the long run it will make things easier for all concerned. We will still accept separate plans for each location, if that is preferred.
5. A set of these plans must be available to all personnel in each licensed location.
